I hereby employ HJALMARSON, SMITH & HANCOCK Attorneys at Law, to represent me in the prosecution of all claims or causes of action I may have against and/or their or my insurers or any other person or entity found liable for my injuries and damages which occurred on the ______ day of ______________, 2006, I hereby authorize and empower my said Attorney to prosecute all claims to settlement or final judgment.

In consideration of the services rendered, and to be rendered in the prosecution of my claims or causes of action, I hereby set over and assign to said Attorneys as an assignment with and undivided interest in said claims or final judgment of:

30	percent interest of the entire amount recovered if collection or settlement of any claim is made before suit is filed;
35	percent interest of the entire amount recovered if collection or settlement of any claim is made after suit is filed, but before an announcement of ready for trial is made; and
40	percent interest of the entire amount recovered if collection or settlement of any claim is made after suit is filed, and an announcement of ready for trial is made.

I understand that my Attorneys will be reimbursed, from my share of proceeds, any expenses, including but not limited to Court related costs, deposition expenses, investigator and interpreter-fees, and the fees of expert witnesses, consultants, and other witnesses, that are advanced by my attorney in the prosecution of my claim.

It is further understood and agreed that if my Attorney should be unable to recover anything, either by settlement or by trial, I owe them nothing. No settlement of my claim or cause of action shall be made without my full knowledge and approval.

It is agreed that any additional attorney fee recovered by my Attorneys is the exclusive property of my Attorneys and is to be considered compensation for his services in addition to the said percentage interest I have assigned to them.

My Attorneys shall have the unilateral right to withdraw from representing me with or without my consent, but Court’s consent of same if required.

I specifically authorize and empower my attorney’s to sign my name to all instruments necessary to perfect or close any claim or suit, or any other instrument that I could legally sign for the purpose of the prosecution or settlement of my claim.
